The progressive nature of hair fall in the future is also kept in mind. The two most popular hair transplant procedures are follicular unit transplantation (FUT) and follicular unit extraction (FUE).
FUT is the more “classic” method. It involves removing some skin from the back of your scalp where there’s an abundance of hair, removing the follicles from that strip of skin, and then reinserting the hair follicles into the part of the scalp where one is experiencing hair loss. The width of the strip varies from 1.5cm to 2cm and length varies from 12cm to 20cm. The donor area is then closed in layers, using a trichophytic method of closure to ensure a barely visible scar post operatively. In FUE, hair follicles are removed directly from the scalp and transplanted to the bald parts of the scalp.
FUE is currently the procedure of choice. In this technique, punches ranging from 0.7mm to 0.9mm in diameter are used to harvest individual follicles. The key to a good result in both techniques is training of the team and proper handling of grafts. Hardly any pain is experienced during the procedure & that too during the period when patients are given local anaesthesia in the donor and recipient areas. Most patients find that once the anaesthesia is given they feel no pain or discomfort during the surgery. Indeed, most patients sleep comfortably during the periods of surgery! Following surgery, patients will typically feel some amount of soreness and numbness, with some mild discomfort such as heaviness. This can be very easily controlled with oral painkillers & almost all patients sleep comfortably at night after surgery. Soon after surgery there is a small dressing which is removed usually by 3-7 days. After the dressing removal, patients can carry on with their routine work. They are advised to start spraying the grafted areas with sterile saline the day after the surgery.
Patients are informed that the initial redness or swelling will subside in a few days time. Small scabs form which the patient should not remove himself by pulling. The donor area heals by 7-10 days or earlier. Gentle shampoo is advised every alternate day for first 3-4 weeks. Strenuous activities are to be restricted till the time the hair grafts fall off. The transplanted hair follicles fall off by 10-14 days or slightly longer, and then hair growth in the transplanted areas starts by about 4 months. It continues upto many months and the final result is achieved by around 12-15 months after the hair transplant. Long term results are expected after hair growth has occurred.
